By making a booking you are agreeing to accept the terms and conditions of service as defined in our "Cosmetic Teeth Whitening Waiver and Release" which can be emailed to you at your request. If you do not agree, you must email us at info@proteethwhitening.com.au to cancel your appointment. A deposit, or full payment prior to service may be required to secure your reservation, and you will be contacted to arrange credit card payment over the phone, if required. Appointments not canceled prior to departure of the assistant to your appointment require payment in full even if you no longer desire the service.

After you choose a date & time, you will create an account with your details and complete the booking.

Your email address and password will then allow you to review, change and add appointments.
